FollowMyHealth® Patient Portal Gives You 24/7 Online Access To:

Manage your personal medical records, communicate with your doctors and pay your SCH Professional Corp bills online.*
*Patients of Walter W. Dombroski, M.D., and Mark Peckman, D.O., only have access to the online bill pay feature.
and select the button at the bottom that says "I need to sign up."
Click the registration link in the email you receive from noreply@followmyhealth.com.
Create your portal login using a unique username and password.
Enter the invitation code provided by your medical organization and click "Agree" to the release of information.
You will receive two e-mails:
One welcoming you to FollowMyHealth® & one confirming your patient connection to the SCH Professional Corporation.

Call your provider's office and ask to schedule a Telehealth Appointment. (Use ON DEMAND for Immediate Care Visits.)

Your provider will send a Telehealth Invite to the email linked with your FollowMyHealth® Patient Portal account.

Click the link in the email and follow the instructions.

Stay in the virtual waiting room until your provider appears on screen.
NOTE:
You must already be signed up with FollowMyHealth® Patient Portal in order to start a Telehealth Call with your provider.
We Now Offer ON DEMAND Virtual Visits:
This telehealth service that you to talk with a provider from SRMC Immediate Care at Firestone Farms using your FollowMyHealth® Patient Portal account.
On-Demand Virtual Visits are available to patients age 5 and older:
Monday - Friday      ?? a.m. - ?? p.m.
Saturday - Sunday  ?? a.m. - ?? p.m.
Not available on Major Holidays
to start an ON DEMAND Virtual Visit or to learn more.
This service connects you with a provider affiliated with SRMC Immediate Care and does not connect you with your normal provider.
