Copies of your SRMC medical records may be obtained by visiting our Medical Records department. A signed authorization form and photo ID, such as your driver's license, is required at the time of your records pick-up. For your convenience, you can download and complete the authorization form prior to your visit.
Those visiting the Medical Records department should enter through the hospital’s Parking Deck, Walkway or Emergency Department, and will be taken to a restricted area, accessible from 7:30 a.m. – 5:30 p.m., Monday-Friday. During the weekends, limited access to medical records will also be available. All people entering the building will be screened for respiratory illness symptoms and travel or exposure history upon entry.
If you are unable to visit the hospital's Medical Records department, you may mail or fax a written request for your records, along with the signed authorization form and copy of your photo ID.

There is no fee for records released to a doctor's office or a medical facility. For personal requests for records, please contact our Medical Records department at 330-332-7536 for fee information.
Most requests will be processed within 10 business days. Urgent requests will be handled with priority.
Monday - Friday: 6:30 a.m. - 6 p.m.
Saturday: 6:30 a.m. - 3 p.m.
